Sleep and Shift Work: Strategies for Healthcare Workers and Other Shift Workers

For millions of people, the workday does not happen between 9 AM and 5 PM.

Nurses, physicians, physician assistants, paramedics, firefighters, police officers, hospitality workers, transportation workers, first responders, and many others routinely work evenings, nights, early mornings, or rotating schedules.

Shift work keeps our communities running around the clock. Unfortunately, the human body was not designed around a constantly changing work schedule.

Working when the body expects to be sleeping—and trying to sleep when the body expects to be awake—can disrupt our natural circadian rhythm. Over time, inadequate or poorly timed sleep can affect alertness, mood, metabolic health, cardiovascular health, and overall well-being.

For shift workers in the Coachella Valley, protecting sleep deserves the same attention we give nutrition, exercise, and other healthy behaviors.

Why Is Shift Work So Hard on Sleep?

Deep inside the brain is a biological clock that helps regulate when we feel awake and when we become sleepy.

This approximately 24-hour cycle—our circadian rhythm—responds strongly to light and darkness.

During daylight hours, the body receives signals promoting alertness. As evening approaches and light exposure decreases, the body begins preparing for sleep, including increasing production of the sleep-promoting hormone melatonin.

Night-shift workers essentially ask their bodies to do the opposite.

They need to remain alert when their circadian system is promoting sleep and then try to sleep during daylight when their biological clock is encouraging wakefulness.

This creates circadian misalignment.

The result can be shorter sleep duration, fragmented sleep, excessive sleepiness at work, difficulty concentrating, and a feeling that the body never completely adjusts.

Shift Work Is More Than Feeling Tired

Occasionally feeling tired after a night shift is expected. Chronic sleep disruption, however, may have broader health implications.

Research has associated shift work—particularly night and rotating shift work—with increased risks of several health problems, including metabolic and cardiovascular disease.[1-3]

Sleep deprivation can also impair reaction time, attention, decision-making, and performance.

For healthcare professionals and others working in safety-sensitive occupations, this becomes particularly important.

Fatigue does not necessarily disappear when the shift ends. A severely fatigued worker may still need to drive home, care for children, prepare meals, or complete other responsibilities.

Protecting sleep is therefore not simply about feeling rested.

It is part of protecting health, performance, and safety.

Start by Protecting Your Sleep Opportunity

One of the most important strategies for shift workers is treating sleep as something that must be intentionally scheduled.

Adults generally need at least seven hours of sleep per 24-hour period, although individual needs vary.[4]

Shift workers may need to think about that sleep differently than traditional daytime workers.

Some people can obtain most of their sleep in one daytime period after work. Others may benefit from a combination of a primary sleep period and a planned nap.

For example, a night-shift worker might sleep for several hours after arriving home and use a planned nap before returning to work.

The exact schedule will depend on work hours, commute, family responsibilities, and individual sleep patterns.

The important principle is simple:

Do not leave sleep to whatever time remains after everything else is finished.

Put sleep on the schedule.

Use Light Strategically

Light is one of the strongest signals controlling the circadian clock.

That means shift workers can use light exposure strategically.

Bright light during the night shift may help promote alertness. Conversely, reducing bright light exposure after work may make it easier for the body to transition toward sleep.

For someone driving home after sunrise, reducing unnecessary bright-light exposure after leaving work may be useful. Once home, the sleeping environment should be kept as dark as possible.

Blackout curtains, an eye mask, and minimizing electronic light can help create an environment that tells the brain:

This is sleep time—even if it is daytime outside.

Light strategies should be individualized, especially for rotating workers whose schedules change frequently.

Build a Daytime Sleep Environment

Daytime sleep has another challenge: the rest of the world is awake.

Traffic is moving. Phones are ringing. Deliveries arrive. Children are playing. Lawnmowers start.

Your bedroom needs to compensate.

Try to make the room:

  • Dark

  • Quiet

  • Cool

  • Comfortable

  • Free from unnecessary interruptions

Blackout curtains can be particularly helpful. Earplugs or white noise may reduce environmental sounds.

Family members may also need to understand that daytime sleep after a night shift is not a nap or leisure time.

It is your nighttime.

Protect it accordingly.

Use Caffeine With a Plan

Caffeine can be useful for improving alertness during a shift, but timing matters.

Consuming caffeine early in the shift may help with alertness. Continuing to drink coffee or energy drinks near the end of the shift, however, may interfere with the ability to sleep once you get home.

Caffeine has a half-life of several hours, meaning a meaningful amount may remain in the body long after consumption.

A practical approach is to use caffeine strategically during the earlier portion of the shift and reduce or stop it several hours before planned sleep.

More caffeine is not always better.

The goal is alertness when you need it without sacrificing the sleep you need afterward.

Naps Can Be a Useful Tool

Planned naps can be valuable for some shift workers.

A short nap before a night shift may improve alertness, particularly when someone has been awake for many hours before starting work.

Some workplaces also permit planned rest periods during extended or overnight shifts.

However, naps should supplement—not routinely replace—adequate total sleep.

Workers should also recognize sleep inertia, the temporary grogginess and impaired performance that can occur immediately after waking.

If you need to drive, make clinical decisions, operate equipment, or perform another safety-sensitive task, allow yourself time to become fully alert.

Think About Meals and Exercise Too

Shift work affects more than sleep.

Working overnight can disrupt meal timing, exercise habits, and other routines that support metabolic health.

Try to maintain reasonably consistent eating patterns rather than grazing continuously throughout the night.

When possible, prioritize foods built around:

  • Vegetables and fruits

  • High-quality protein

  • Whole-food carbohydrates

  • Healthy fats

  • Fiber-rich foods

Heavy meals immediately before sleep may make sleep more difficult for some people.

Physical activity is equally important.

Regular exercise can support cardiovascular health, metabolic health, stress management, and sleep quality. The ideal time to exercise varies between individuals, but extremely vigorous exercise immediately before planned sleep may be stimulating for some people.

Sleep, nutrition, physical activity, stress, social connection, and avoidance of risky substances all interact.

Be Careful With Alcohol and Sleep Medications

Alcohol may make someone feel sleepy, but it is not a good sleep strategy.

Alcohol can disrupt sleep architecture and contribute to fragmented, lower-quality sleep.

Similarly, routinely relying on over-the-counter sleep aids or prescription medications without addressing the underlying sleep schedule may create additional problems.

Melatonin may be helpful in certain circumstances involving shift work and circadian timing, but dose and timing matter. It is reasonable to discuss its use with a healthcare professional rather than assuming that taking more will produce better sleep.

Sleep medications and supplements should never substitute for addressing the fundamentals of sleep timing, light exposure, and the sleep environment.

What About Rotating Shifts?

Rotating schedules can be particularly difficult because the circadian system may never have enough time to adapt.

If you have some control over scheduling, consistency can help.

When rotating shifts are unavoidable, strategies may need to change depending on how frequently the schedule rotates.

This is where individualized planning becomes particularly valuable.

A healthcare professional familiar with sleep and circadian health can help review work schedules, sleep timing, caffeine use, light exposure, naps, medications, and symptoms.

When Shift-Work Sleep Problems Need Medical Attention

Not every tired shift worker has a sleep disorder.

However, persistent insomnia or excessive sleepiness associated with a recurring work schedule may represent shift work disorder, a circadian rhythm sleep-wake disorder.

Shift workers can also have the same sleep disorders as everyone else.

Symptoms such as loud snoring, witnessed pauses in breathing, waking up choking or gasping, persistent insomnia, severe daytime sleepiness, restless legs, or falling asleep while driving deserve medical evaluation.

Do not assume that every sleep problem is simply “because I work nights.”

Sometimes there is another treatable condition underneath it.

Shift workers cannot always change their work schedules.

A nurse cannot simply close the hospital at bedtime. A paramedic cannot tell emergencies to wait until morning.

The goal is therefore not perfection.

The goal is to identify the parts of sleep and health that can be controlled.

Protect your sleep opportunity.

Create a dark and quiet sleeping environment.

Use light intentionally.

Time caffeine strategically.

Use naps appropriately.

Exercise regularly.

Pay attention to nutrition.

Avoid using alcohol as a sleep aid.

And seek medical evaluation when sleep problems persist.
